*NOTE : With respect to entries at Sl. Nos. 22,23,50 and 58, the exporters shall furnish a certificate from Department of electronics or from the jurisdiction Central Excise authorities stating that the export product has been manufactured by them in India. However, if the exporter furnishes AR-4 form, no separate document should be insisted upon.
